Usually a recording that's too short, too quiet, too noisy, or that has music playing behind the voice. If a voice profile fails, the credits it used are returned to your balance automatically.
A failed profile can't be retried in place — start a new one, and it will use credits again. For a good result: quiet room, phone close to your mouth, no backing track, at least 30 seconds of continuous speaking or singing.
